About this course

Most people who write nursing exam items already know clinical content. The problem is they don't know the rules that govern how a valid NCLEX-RN item is built. A question can be clinically accurate and still fail review because the stem is ambiguous, the format doesn't match the cognitive process it claims to measure, or the option count violates NCSBN structure. This course closes that gap. You'll work through the full 2026 NCLEX-RN Test Plan, including the eight Client Needs categories, their percentage distributions, and the six Integrated Processes that thread through every item. From there you'll move into every item format the exam uses: the seven traditional types (Multiple Choice, SATA, Calculation, Ordered Response, Hot Spot, Graphic Options, Audio) and every Next Generation NCLEX format (Extended Multiple Response, Matrix, Drop-Down, Drag-and-Drop, Highlight, Bowtie, Trend). For each one, you'll see the structural rules, the exact NCSBN stem language, and what a non-compliant draft looks like versus a fixed one. The NGN section goes one layer deeper. The NCLEX doesn't just test knowledge of clinical facts. It tests clinical judgment, which means items have to target specific cognitive processes from the NCJMM. You'll learn to use the selection matrix to pick the format that best matches what you're actually trying to measure, and you'll practice applying a six-step review workflow to AI-drafted items. By the end, you'll be able to take a draft item, identify exactly what's wrong with it, and fix it to standard.
